vCare’s mission is to make holistic, connected and sustainable health care for chronic patients accessible.

vCare’s service is a disease management platform that provides remote personalised care plans to patients. The system maps the relevant factors that have a positive effect on the individual and adapts the motivation accordingly. It connects the parties involved through a patient-oriented app, an app for the clinics and an app for relatives. The overall knowledge of the connection between multiple aspects, motivation and adherence are used to make predictive analysis and preventive intervention in the citizen’s course via AI and ML.

Our ambition with vCare is to reduce transport, optimize the use of existing healthcare resources and provide equal access to care irrespective of geography, helping reduce the burden on hospitals and increase the quality of life of patients.
